Where To Make Bug Reports + Some Miscellanea

I've been going through my review copy (neener! neener!) and have been having some weirdness with my video. I'll describe the problem, but would like to know where the best place to report bugs is in the future.
When the game starts up, the monitor turns off! (mine shuts off if it stops receiving a video signal from the computer). When the game starts, the screen goes black and the monitor tells me the session has been stopped and proceeds to shut down. However, if i click or press some buttons, it comes up again in time for the monitor to not shut down. This also happens when i select "Credits" from the main screen: i hear some music and then the screen blanks out and monitor goes into shutdown mode. If i click anything though, the credits stop rolling and it send me back to the main screen. Aarrrgg!!! Sometimes it does it, sometimes it won't. (that's the last thing a debugger wants to hear, i know. sorry).
And out of curiosity, i did get the credits to roll once and saw David Craft in the special thanks list. Is this David "Stormhound" Craft of MOO3 fame and how does he factor in? I asked him and he's pretty surprised about it. I'm assuming it's not the same person?
