Here's the number 5 icon. It's the My Music Folder.
The folder's model and texture is the same as the open folder icon.
The Addition here, is the music note.
Model:
I used the sphere tool to create the bottom part of the note. I used the capsule tool for the stem of the note. And I used the spline tool for the shape of the tip of the note. Then, I beveled the tip.
Texture:
A modified version of the folder texture, only using purples instead of the yellows.
Lighting:
You'll find I added a third light, which excludes everything except the music note. This was done to match the lighting in the Windows icon.
