Here's the number 15 icon. It's the My Pictures icon.
The folder's model and texture is the same as the open folder icon.
The Addition here, is the picture.
Model:
I used rectangle shape tool to create an 85 by 65 rectangle. I then beveled it. Finally, I edit poly modifier to bevel the face of the picture.
Texture:
A modified version of the folder texture. However, I changed the colors to light blues and added an additional material to the composite to include the picture.
Lighting:
There's also additional omni and directional lights.
