Normally, you would do the following:

1 - If IconPackager is in use: Start IconPackager > Tools > Restore all Icons > click Yes to verify > click OK to apply changes (which will also close Icon Packager after a few seconds).

2 - Repeat step #1 for the Restore all Cursors, and then for the Rebuild Icon Cache options found in the Tools menu.

3 - Unload WindowBlinds (using either the configuration panel unload option, or by right-clicking the WB system tray icon and selecting the unload option).

4 - Open Display Properties > click Themes tab > select Windows XP from the Theme drop-down list > click Apply button to restore Windows XP default theme (if the apply button is not available, toggle Windows XP selection by selecting Windows Classic, and then reselecting Windows XP).

Uninstall WindowBlinds if that is your decision, and then reboot the machine.

After reboot, you can remove the skins by going to "C:\Program Files\Stardock\ObjectDesktop" folder, and deleting the WindowBlinds folder.

Note - if you uninstalled WindowBlinds without either unloading it, or restoring the Windows XP theme completely, you may have had a ".dll" hanging on to something which may create issues.

For the icons, you may need to use the custom (or Icon) tab in the relevant icon's properties to restore the proper image. If there is not a "restore default" button, you will need to browse to the proper ".dll" or ".exe" file to select and apply the correct icon.

Without more detailed information, this is all I have at this time.

Someone else may have more on why your icon changed to one you have never seen before.

I hope this helps.

Also, for the icons - you can also right-click the file > select "Choose program"  > select the "Always use selected program to open this kind of file" (if you want to assign a default program icon) > click "OK".
