Short answer: You need the planet's entity, mesh, and texture files, you'll need the entity and any other related files for their effects. Put those in your mod. Then you'll have to copy the sections in the galaxy scenario def file for the planets that you've added and adjust the section counts accordingly. In your game's main folder there will be dev exe's for each version of the game. Quickest way to get there is (if you're running anything newer then XP) right clicking on your existing short cut to the game (even if it's in the start menu) and selecting the option 'open file location' in the context menu that pops up. You'll see the developer executables in there. Right click on those and select create short cut, and you can move those short cuts

Black holes can still be used as stars on custom maps using the forge or the in-game map creator. See black 5 for example. They were removed from the random settings because as Ryat said, the AI treats them as they do every other star and takes the shortest route through ultimately costing them their fleet. It's actually very easy to scale everything to approximately the right size and it isn't really all that much work... I believe Teal had created a little utility to easily resize any planet desired a couple of years ago. The problem is the time involved to actually play the game at the proposed scale. That aside, my largest concern would be the outer galaxy bubble that displays all of the stars and skyboxes. I'm not sure how dynamic it actually is, if it
