You definitely need to speak with a helpful third party, that is, some kind of therapist. You have a variety of important issues that need sorting out before making any big decisions. I know it is difficult, but I agree that you must face this problem (preferably with your spouse). But, immediately, get help so you don'tg hurt yourself (or anyone else).
