Go into Display Properties/Appearance (and then, on XP, Advanced/Other Advanced Controls). Click the image of the active titlebar on the dialog that appears and reduce it's size a few points with the controls below the image.

I don't know of any way you can, currently. I'm not sure I'd personally want it to be, anyway (if your WB skin causes each window to crash, you'd *really* like to be able to bring up the WinSec panel without having it skin, right? *g*) Of course, I might be proven wrong in the next ODNT update.

Yes, it's reasonably safe, although it's probably best if you don't mess with any of the other settings there - most are just ones in the WB control panel, but some are special debugging ones that you don't normally need. Keep a backup if you're really paranoid.
Often this happens when a) the download is corrupted, or b) the download gets deleted before you load it. To try and counterract these, use Save As... options to save a skin to a file, then load via the Skins panel Add button and download using the text download link rather than the skin image link (I've found it sometimes works better).
Norton Utilities are not skinned because they do things in a non-standard way which cause bad things to happen when skinned. Display Properties is (currently) unskinned to ensure you have a way out if a skin is so bad that it crashes every window you display - usually because of a buggy DLL. You can change this by adding SkinDisplay=1 to the [Global] section of wb.ini in your Windows directory.

I use PSP, pretty much nothing else. Usually, when I manage to get something that I think is cool, it takes a lot of work, with fiddly little changes to make it look smoother. Often I try things, and they don't work. Sometimes you just have to accept that and move on - you usually find an even cooler idea down the road. Heh, sorry, didn't refresh the topic. If you have merely "registered" the shareware version, that just stops it bugging you. You have to uninstall the shareware version and install the registered version (you should have had a link sent to you in you registration email). Only then will scrollbars be skinned - the shareware version just doesn't contain the code to do it.
If you're running the trial version, that doesn't skin scrollbars. They're reserved for the registered version.

Notepad, Paint Shop Pro, Sound Recorder. SkinStudio is too big for my screen (800x600) and gets in the way of quick edits.
I don't want to update my skin to fix small things because of this - I'm worried it'll make me look like I'm "hogging" the new updates board. I would like an option to *not* place it in that section when I update a skin.
Yes, the XP file is just an extra UIS (or rather, INI) file, both for separation purposes and because it might be pushing the limit on file sizes for 9x-based computers if they were kept together. The WBD files are a "native" format of skin information - they're so WindowBlinds doesn't have to have read the UIS file every time it wants to start a program. And yes, they are annoying. There doesn't appear to be a way to "upgrade" a WB skin to the WB-XP section. I think this is going to be something a lot of people are going to want to do when the new version of SkinStudio comes out, if not before.
SkinStudio coder Adam Najmanowicz has just completed his MSc (congratulations to him, BTW). I suspect he'll be working on implementing the new features immediately.

Also, a downloadable trial version may be available "soon". Hopefully at that point you'll be able to buy it standalone, too - but if you want the most regular updates (and like testing the newest features), or access to other programs by Stardock, an ODNT subscription is good value.
