You didnt understand the approval system. Morale is a fixed value, that doesnt change over time. Your population changes (growth) over time. Approval = Morale / Population. To keep your approval (and the corresponding boni) high, you need to invest in morale via buildings or tech. You should try to have the morale as high as the max population of your planet. I agree with points 3) and 4)

There are some mistakes in the patch notes: - Hot springs are listed twice. They actually give +2 morale now - Port of call gives +1 tourism income, not +3 as listed - Xenophic is listed twice in the patch notes. With smaller steps (not 9 or 16 population per 1st level city), morale management would get much easier. I think it would be healthy for the game to have at least one more level. Maybe +4/+9 for silics and +8/+16 for carbon and aquatic.
