i have seen this as well when something else overrites an image in a theme/skin. happens. Clearing the cache is a great idea.

Its always a good idea to group things together if possible, but sometimes things are grouped inside other things, and that can complicate it more. I like using the locked or no activation methods as well, use them more and more these days. All great suggestions.

I have written 3 responses to this and deleted them all.
