I think it has a lot to do with understanding. For many people, they don't really understand a situation unless they have experienced something similar. I know that I never had empathy for people with depression before I experienced it firt hand. I just didn't truly understand the nature of the thing. This may be true for a lot of people, for others it may be simple selfishness.
