If you have downloaded a ".dockzip" file, simply double-click on it. If you have downloaded a zipped folder (.zip) then extract the folder into one of these two places: "C:\Program Files (x86)\Stardock\ObjectDock\Backgrounds" or "Documents\Stardock\ObjectDock Library\Backgrounds" Once you have done that, open your OD config, go to the "Appearance" tab, and select your new background from the drop-down list.

The free version is more limited. You can only have 1 dock active, and you don't have the option of tabbed docks. Plus lets you have multiple docks, tabs, system tray (as long as you're not using a 64-bit system). I highly recommend buying plus. Here's a link that might help you. What I usually tell people is: a skin is something that makes my Windows on the computer look cool and unique. That explanation along with seeing my screen usually gets the point across.
