End the game as soon as possible. Harass him early-game so he can't get experience. Make sure he doesn't get any kills(the only way to afford ashkandor without the game lasting forever). Make sure that you can finish the job when you buy giants, so you don't feed. Cap their portal flags at the sides to speed this up and lock them. I hope this helps, because this is the best advice I can give.

This seems like more of a support guide, where you are dependent on having good teammates to cover you and vice versa. If you have a good team this build could work well, but in a 1v1 game or a match where your teammates aren't there to cover you you'll probably end up being dominated for most of the match. An Oak without shield or pentinece will have almost no choice but to run from an assasin or a general with an assasin build.

The only map I really dislike is Crucible. There's only one way out of your base, so an agressive enemy(or a tower Rook) can make it impossible for you to leave your base for a comeback if you're losing.

This is probably an oversight by the dev. But the benefits of an infinite mist seem to be offset by the negatives, such as you being unable to push lanes. Still, this could be fixed by having a time limit on how long Erebus can remain in mist form if it's really that much of a problem. I've never faced this dilemma, so I don't know how troublesome it can actually be.

Two days ago I played a fortress match against a Rook with a Queen of Thorns as an ally on Crucible. QoT would bramble shield him so he could set up towers and attack our defenses without taking damage. He set up a group of towers in the lane leading out of our base so we couldn't get out. It was ridiculous, although my ally and I stuck it out through the whole match in hopes of a comeback. Naturally, it didn't come close to happening.
