Had the same bug - i moved my milita out of city ones, but after re-starting and re-loading all was fine. Had also a bug that after battle one of my stationed units got 12 soldiers there (instead of 4) and i could not disband or move out this unit, had also an error that during tactical battle my sov was nowhere to be found on a field of battle but still could cast spells from outside of area. All this bugs happened with 9 troops in a city. Still, very rare bugs.
I played couple of Iron Man but annoying number of bugs forces me to save frequently, not to mention crushes on late game .... Game is nothing but easy on ridiculous up, though some exploits make it plausible to win.

Even though i have a cleric after each battle i have to take couple of turns in the city to recover which slows the game a lot. Any comments on other points?

My impression/bugs list after playing through 200+turns on insane world/expert Ai's/large map/all dense/terrifying monsters: - palace - messed up tooltip info: faction prestige -20 -> +1 - scarce gold - I have right now 8 cities, and taxes on medium in order not to go into negative. I never upgrade units, it is cheaper just to spend 2 turns producing new unit with all the bonuses of a fortress than to pay 75g for each unit's upgrade. It will cost me couple hun
Confirming the bug. Outpost should not be razed, since they provide now bonuses to army there is a reason why you want to keep them close to a city. There is generally problem with borders, and it is a loooong time problem. They do not calculate well, for instance, you can build the city right next to AI' borders and they will not move an inch, but if they build anything close to your border than you are in big troubles.
I started to play a game on insane world, but on large map with good number of AI's. I am playing Ythril, and i was on a good start - got 7 cities around turn 200. But i am slowly getting discourage, killing those over 1000hp enemies takes forever ... I am now fielding Yaggernauts which kind of help but still ... Turtling a lot. Just hoping that my development eventually will outsource the world, sometimes i wander, what if i field the hardest units and still, my enemy is way ahead of me
Serious bug (let me know if it vanilla, i will report it): if i build a outpost close to a city that it gets incorporated into the city then this outpost cannot be upgraded. You can click upgrade, choose whatever you want, upgrade will be finished but will not show on the outpost screen.... You can build them without end :)

Are you using Catapults to siege the city before the battles? You need Blizzards, Fireballs, Catapults, radius damage staves to siege. If you have cleared the entire map before you got to those, that means the pacing of lairs is way off. I am working on some more challenging stuff for that stage of the game to keep things interesting. My world difficulty is expert. Also fallen dragon does not produce anything. I have two bear lair on outskirts of my kingdom, nothing coming from there as well. Anyway, i will have to start to new game, b/c this one i will not win. I have a war which neither of us can win, since it is quite difficult/practically impossible to conquer city. i think with lairs should be the same it is with a player right now. If i attack enemy and win, i remain on tile from which i attacked. Monsters defending lairs should be able to attack anything in their vicinity, but not to move out of their lair following the successful attack. I do not know though whether game engine permits it though ...
